Wufoo is a form builder application. The name is the dumbest thing I ever heard but the software is pretty neato. I created the above form in less than 3 minutes (including signing up for an account). I think this sort of thing could be very useful for small businesses without a dedicated web coder.

Microsoft’s Visual Web Developer 2005 (freeware) for the .NET 2.0 platform has this, as well as many other canned web components, as part of their IDE. It’s laughable how easy it is to create fully functional database apps with logins, themes, database connections, etc. all without typing a single character.
I am not a M$ groupee, but after using VWD 2005 at work, I have to admit it is very, very impressive and powerful.
